RFQ process mortuary equipment purchase order W-9 AMC procurement

All AMC and USPE equipment is quote-based — no public pricing. This post explains how the procurement process works from initial inquiry through delivery, including what documentation is available for institutional buyers.

Step 1: Submit Your RFQ

Use the AMC RFQ form to submit equipment specifications, quantities, and delivery requirements. For mortuary coolers, include floor plan dimensions and ceiling height. For embalming tables, specify required load rating. For first call cot packages, specify cot count and weight ratings. The AMC procurement team receives your RFQ and begins quote preparation immediately.

Step 2: Receive Your Quote

AMC responds with a consolidated quote within 24 hours of RFQ receipt. The quote includes item pricing, lead time for each category, shipping estimate, and any installation notes relevant to your configuration. For walk-in cooler systems, the quote includes rough-in dimension requirements.

Step 3: Request W-9 or Procurement Documentation

Government agencies, hospital systems, universities, and coroner offices requiring W-9 or bid documentation before PO issuance can request it at the same time as their quote or separately via the W-9 request page or email to po@mymortuarycooler.com. See: RFQ Center | W-9 | Purchase Orders Hub.

Step 4: Issue Purchase Order

Submit your purchase order via email to po@mymortuarycooler.com or through the RFQ form. AMC accepts POs from funeral homes, crematories, hospitals, coroner and ME offices, government agencies, universities, and removal services.

Step 5: Production & Delivery Confirmation

AMC confirms production start and estimated delivery upon PO receipt. Lead times are confirmed at quote before any production commitment.
